The Companion Animal Parasite Council (CAPC) — the nation's leading source on parasitic diseases that threaten the health of pets and people — today issued the CAPC Top 10 Cities Heartworm Report for the month of Febuary with Salt Lake City, Utah, topping the list. In its mission to continuously monitor and report emerging threats to companion animals, CAPC recently launched its new monthly report initiative to alert pet parents, veterinarians, and pet-related service providers about U.S. metropolitan areas experiencing the highest percent increases in positive heartworm tests.

"What the new CAPC Top 10 Cities Heartworm Report demonstrates is that heartworm disease — transmitted by mosquitoes — is a national threat to pets who are increasingly vulnerable to this debilitating and potentially fatal illness in most communities across the country," said Dr. Michael Yabsley, CAPC board member and professor in the Department of Population Health, College of Veterinary Medicine and Warnell School of Forestry and Natural Resources at the University of Georgia. Pets in cities on this month's CAPC Top 10 Cities Heartworm Report may have been exposed locally or travel-related exposure may have been a contributing factor to the increase in positive tests. Other factors include transporting of shelter dogs, pet owner compliance in administering monthly heartworm preventatives, changing weather patterns and mosquito microclimates.

Nationally, prevalence rates for heartworm have risen each of the last five years and are now up 20% from 2013 levels, according to CAPC. "Many pet owners mistakenly think their dog or cat isn't at risk for heartworm because they don't live in what has been historically considered a heartworm 'endemic' region of the country. "This is no longer the case," said Dr. Cassan Pulaski, DVM, MPH, CAPC board member and Merck Resident in Veterinary Parasitology, School of Veterinary Medicine at Louisiana State University. "While southern regions of the country have historically been associated with heartworm, we now know pets all over the country are potentially at risk for heartworm disease throughout the year." Just one heartworm-infected dog increases heartworm risk
When a mosquito bites an infected dog, it transfers the microscopic heartworm parasite (larvae) to the next dog or cat it bites. It only takes one heartworm-infected dog to substantially increase the number of infected mosquitos that can transmit heartworm parasites. This was clearly demonstrated in a study published in the Journal of Medical Entomology which found over 73% of mosquitoes collected inside the kennel of just one heartworm-infected dog, demonstrating that "a single heartworm-positive dog potentially increases infection pressure on susceptible animals sharing mosquito exposure." Mosquito microclimates
Regardless of where you live, the risk for heartworm is ever-present due, in large part, to mosquito microclimates. "No matter the temperature, mosquitoes can thrive in sewers, stormwater drains, crawl spaces, alleys and other warm spaces where they survive and feed through winter months," said Craig Prior, a veterinarian and former owner of VCA Murphy Road Animal Hospital in Nashville, Tennessee, and past president of the CAPC board of directors. "There can be two feet of snow on the streets, but between high-rise buildings, it may be 50 degrees and wet —a perfect environment for mosquitos to breed."

Increasing travel with pets
Heartworm disease has likely become more widespread in the U.S. Due, in part, to pet owners who are increasingly traveling with their pets — exposing them to varying, unpredictable temperatures and heartworm-positive dogs. According to a 2017-18 American Pet Products Association survey, 37% of pet owners are traveling with their pets every year – up from 19% a decade ago. Transporting rescue dogs
Another reason heartworm disease has become more widespread in the U.S. Is likely due to increased transportation of rescue dogs from regions with higher heartworm prevalence to areas where the disease was previously uncommon. Due to financial and time constraints of rescue and shelter groups — particularly during natural disasters —many animals are not protected, have missed a monthly dose or have not been tested or treated for heartworm. While CAPC supports efforts to find every rescue dog a home, pet owners need to understand that imported dogs may be unwittingly infected with heartworms and become a reservoir of infection in their communities.

In 2017, for example, more than 31,000 rescue dogs were transported from southern U.S. states to Colorado where they're considered to have a higher likelihood of being adopted.

"In an increasingly mobile society, pets are vulnerable to heartworm disease in every community," Prior added. "The neighbor next door may have adopted a new rescue dog and, despite good intentions, hasn't yet had it tested for heartworms. Meanwhile, it may be heartworm positive, posing a high risk of exposure to your pet."

Not worth the risk
"Most people consider their pets as family members and wouldn't knowingly expose them to infection with a potentially fatal disease that can ultimately compromise the length and quality of their lives. Yet millions who fail to protect their pets every month from heartworm infection are doing just that," Prior added. "The risk just isn't worth it. Heartworm preventatives are affordable, safe and effective. This is why CAPC recommends all pets, no matter where they live, be tested annually and placed on heartworm preventatives 12 months of the year."

In addition to the cities in the CAPC Top 10 Heartworm Report, many other communities also are experiencing increases in heartworm prevalence. Casco Manufacturing Solutions: Five Traits of a Mattress that Promotes Patient Healing

Patients face many medical challenges throughout their continuum of care, but starting with better mattress quality can provide comfort, stability and speed healing, says a wound care specialist and quality consultant for Casco Manufacturing Solutions in Cincinnati.

Shelly Byington, an R.N. who is wound care certified (W.C.C.) and a Casco quality consultant and healthcare expert, is a traveling nurse who calls on about 30 homebound patients each week throughout mid- to southeastern Ohio.

A 23-year expert in wound care and member of the National Pressure Ulcer Advisory Panel (NPUAP), Byington began her career by caring for senior citizens at nursing homes. She drives an average of 200 to 300 miles per day visiting homebound patients with diabetes, obesity-related diseases and chronic cardiovascular or vascular diseases.

"Nurses everywhere should ask themselves whether their patients' mattresses are helping or hindering healing," said Byington. "Pressure injuries that form from a poor mattress, recliner chair or wheelchair cushion can become chronic or infected and be slow to heal, which can cause increased pain and unnecessarily long recoveries."

Byington created a list of the five most important attributes to consider when selecting a mattress that will help heal—not harm— patients.

Pressure redistribution— The insides of mattresses, recliner and wheelchair cushions are extremely important, said Byington.

A great mattress provides multiple layers with different zones of padding. There should be a support layer on the bottom, with a softer layer in the middle, and a top comfort piece. Mattresses that distribute a patient's weight evenly across the surface of the mattress and include a sloped heel zone for pressure redistribution help eliminate pressure points, which, along with conjunctive therapies, can assist in the healing process, she added.

"This is so important," said Byington. "More than half of long-term care patients have moderate to severe peripheral vascular disease, which puts them at risk for developing lower extremity ulcers."

Fabric—Breathable mattress fabrics using select polyurethane and vinyl are most preferred, said Byington. Two-way stretch fabrics provide added comfort and durability. They also prevent "hammocking" and allow for easier patient transfers.

The best fabrics absorb body heat and perspiration and disperse it away from the body, said Byington. This keeps patients cool and dry and reduces the possibility of skin breakdown, which can lead to pressure ulcers. Be alert to read related news.

Antimicrobial properties— All mattresses, no matter the quality, should offer easy cleaning and maintenance. Chronic wounds and pressure injuries are known to carry a "bio burden," or bacteria, which put the patient at risk for infection. Mattress fabrics should be antimicrobial and hypoallergenic, as well as treated to inhibit bacterial growth.

Seams—Mattress seams should be sealed, not sewn, which decreases the likelihood of body fluids or secretions entering into the mattress core.

Mattresses should be selected according to the needs of the patient, not the lowest price an insurance company often approves, she added. "Obese patients need mattresses that can accommodate from 350 to 500 pounds. Some higher-quality mattresses use interconnected air chambers that provide quiet, non-powered pressure redistribution that allows the mattress to automatically adjust to patient movement. These mattress systems are best for pressure injury prevention and treatment."

Quiet, non-powered mattress technology allows for even-body weight distribution and reduces the risk of skin breakdown. Some mattresses have air chambers that are motorized, but if the motor breaks down or the settings are wrong, pressure injuries can form quickly, she added.

Barbie, The Doll We All Love, Turned 60!

Barbie turned 60 on March 9.

Barbie creator, Ruth Handler, and husband, Elliot Handler, founded Mattel Creations in 1945. She was inspired to design the Barbie doll after watching her daughter play with paper dolls. Most dolls made for children were baby dolls, not fashion models. The first Barbie was introduced at the American International Toy Fair in New York on March 9, 1959. This date is considered her official birthday.

Barbie's look was inspired by Bild Lilli, a German doll based on a comic strip character that Handler bought in Europe in 1956. Mattel purchased the rights to Bild Lilli in 1964. (Bild Lilli dolls from 1952 to 1964 have their own collectors.)

The original Barbie, No. 1 as she is known by collectors, is easily identified in its black and white striped bathing suit. It had copper tubes in its legs and holes in its feet, enabling it to be upright in a stand. No. 2 Barbie, released the next year, no longer had holes in her feet and sported a different wire stand. A Barbie No. 1 with a brunette ponytail recently auctioned for $4,600, nearly double its high estimate. And, depending on rarity and condition, her fashion accessories can sell for more than a vintage Barbie itself.

Barbie has more than 150 careers on her resume and reflects how women's careers have changed since the 1950s. She broke the toy "glass ceiling" in 1960 when she worked as an astronaut and went to the moon.

Barbie was a "Commuter" in 1959 and a "Busy Gal" in 1960, both ambiguous terms for the "Career Girl" (1963) trying to find her footing. Over the years she has been a Fashion Editor (1960), Nurse (1961), Stewardess (1961; she became the Pilot in 1990), Surgeon (1973), and Veterinarian (1985). She entered the Board Room in the 1980s and 1992 as a Business Executive and CEO. Almost Half of LGBTQ Millennials Plan to Have Children Using Infertility Treatment; Fairer Health Insurance Policies Can Help

A new report published by the Family Equality Council reveals a strong interest in parenting among U.S. LGBTQ Millennials compared to previous generations; 77% of those aged 18 to 35 already have or are considering having children, 63% in the 36 to 54 age group and 33% of those aged 55 and older.(1) Among all LGBTQ prospective parents, 41% are considering assisted reproductive technology (ART) such as intrauterine insemination (IUI), in-vitro fertilization (IVF), or surrogacy.(1) Dr. Mark P. Following the 2015 Supreme Court ruling in favor of marriage equality, advocates projected an increase in the number of LGBTQ families.(2) The Family Equality Council survey found that 48% of LGBTQ Millennials are currently planning to have children, compared to 55% of non-LGBTQ Millennials.(1) While only 5% of existing LGBTQ families used assisted reproductive technology, 41% are considering using it to start or grow their families.(1) Yet they are more likely to face obstacles than heterosexual couples; the authors of the Family Equality Council report noted that "insurance policies are rarely created to meet the needs of LGBTQ family building, and discrimination against LGBTQ prospective parents by agencies and providers remains widespread."(1)

"Assisted reproductive technology expands family-building options for same-sex couples and allows them to have biologically related children of their own," explained Dr. Trolice. He is a longtime advocate for LGBTQ parenting rights and insurance equality, and Fertility CARE: The IVF Center is committed to providing compassionate care to all patients, regardless of sexual orientation or marriage status. Reproductive treatments for gay and lesbian couples can vary depending on whether the prospective parents are male or female, but are similar to the choices available to single men and women.

Lesbian couples may opt to have one partner donate the egg and the other carry the pregnancy, which would involve IVF; if one partner will fulfill both roles, IUI or IVF may be used. In either case, they would use donor sperm from a friend or sperm bank. Gay couples require an egg donor and a separate gestational carrier. During the IVF process, the prospective fathers may choose to fertilize the egg with the sperm of one or both partners through a procedure known as intracytoplasmic sperm injection, or ICSI.

With a growing number of fertility clinics welcoming same-sex couples, prospective LGBTQ parents can avoid the discrimination some have previously faced from healthcare providers and adoption agencies.(1) However, the cost of ART remains a challenge not only for those whose health insurance does not cover fertility treatments but for those whose healthcare plans require proof of infertility and/or mandate six to 12 cycles of IUI to be paid out of pocket before coverage for reproductive assistance kicks in.(2, 3)

A single round of IUI can run between $300 and $1,000,(4) or upwards of $1,500 with donor sperm,(3) while IVF can cost tens of thousands of dollars.(3) One study found couples who undergo an IVF procedure spent on average $15,435 more than those who underwent IUI.(4) A USA Today article noted it can cost a lesbian couple $20,000 to $30,000 just to get pregnant, while a male couple could be looking at six figures including a surrogate to give birth to their child.(2)

That's why Dr. Trolice is a vocal advocate of fair and equal health insurance policies for infertile and LGBTQ couples. "Parenthood is a closely held dream for many couples, and employers and insurers have the power to make that dream a reality through equal and inclusive healthcare plans," he asserted. He says that allowing doctor and patient to determine the optimal course of treatment, and pursue IVF as appropriate, can help improve health outcomes and reduce the likelihood of multiples and genetic conditions, thereby lowering overall costs for employers, insurers, and parents alike.
